Plantation Baptist Church AWANA Kids Program Pre-Registration & Consent Form
My child(ren) have permission to attend and participate in the Plantation Baptist Church AWANA Kids Program. In case of emergency, I, the parent/legal guardian of the above mentioned child(ren), give permission to the adult sponsors of Plantation Baptist Church to secure proper treatment for the health and comfort of my child(ren) until I can be reached.

Photo & Video Release:
I give permission to Plantation Baptist Church to photograph and video record my child(ren) during this church programs. I give permission to use the photographs and video for any church publications, church FaceBook page and the church websites.
